What We Do
Strong Children Wellness recognizes that the traditional way of delivering medical care is often insufficient to meet the dynamic needs of children and families—that is why we offer a range of services to address medical, mental health and social needs, and have partnered with established community-based organizations to integrate primary care services into their wide array of existing support services and programs.
Our integrated and virtual care models allow us to expand the range of services that we provide.
Our team includes primary care providers, care managers, family navigators, and behavioral health providers who work with your family to coordinate care and provide holistic support.
Our Services
Advanced Primary Care
We offer on-site and virtual well child and family care, immigrant health services, parenting support, and programs that enhance child socio-emotional development.
Care Management
We screen for social, physical, and mental health needs, and connect children and families with medical, mental health, learning, developmental, and social issues to our care management program. Our care managers work with our community partners, medical and behavioral providers to coordinate health, educational, and mental health services and address social needs such as housing instability, food insecurity, and legal issues.
Integrated Behavioral Health
Our behavioral health providers offer support for emotional and behavioral issues, including anxiety, depression, and ADHD. We also refer to and collaborate with our community partners, who provide therapy and psychiatric care for many other mental health needs.
Community Partner Network
Patients can access on-site mental health and social services offered by our community partners. Our navigators help families access and engage in a range of services, including adult education, employment services, benefits assistance, childcare, and after-school programs.
Contracted medical care services
We provide contracted medical care services for community organizations serving youth with severe psychosocial complexity, including foster care agencies and residential treatment facilities.
