Community-Based Partnerships
Strong Children Wellness partners with established community-based organizations to build a culture of health and to co-create innovative models of integrated, collaborative care. As partners, we leverage our respective expertise, programs, and services and tailor them to the unique needs of the families we mutually serve. We have a track record of improving engagement in physical, mental health, and social care, reducing unnecessary emergency room use, and improving health, developmental, and mental health outcomes.
Health Plans
We work with insurance providers to provide high touch, coordinated wraparound care to their most costly members. Our interdisciplinary care team works in deep collaboration with social care providers to help members receive comprehensive preventive primary and mental health care, identify and address social needs, and avoid unnecessary hospitalizations and trips to the emergency room.
Contracted Primary Care and Wellness Services
Strong Children Wellness provides contracted primary care services to organizations that serve highly vulnerable populations. Our providers are expert at working with children in foster care, adolescents and young adults with serious mental illness, immigrant populations, unhoused families, and justice-involved youth.
